	This field contains the prescriber's unique 
	identification number. CMS will transition to
	the use of the NPI when it is implemented. In
	the interim, CMS requires use of a DEA number
	whenever it uniquely identifies the prescriber
	and is allowed by State law. In other cases,
	the prescriber's State license number or UPIN
